　　申请波兰商标注册所需文件

  

       注：所有文件必需以英文书写/显示，如非英文，则必须提交经公证的英文译本。


　　1、简单的委托书;

　　2、申请人的姓名，国籍，职业和地址;如申请人属合伙公司，则需提供所有合伙人的个人资料。如申请人属法人团体，则需提供注册证书或商业登记证或营业执照。如申请人在波兰有营业地址，则需提供详细的营业地址。

　　3、拟使用该商标的商品或服务和相关类别(根据国际分类)。

　　4、关于拟注册商标的资料：

　　1)如拟注册的字眼不是英文，需说明该字眼所属语言和英文译本;

　　2)如拟注册商标是图案，则需简单解释该图案和10份图案样本;

　　3)如属三维图案，则需提供以二维和多角度制作的图案样本多份;

　　4)如拟注册商标饱含商品样本或包装样式，需提供该商标最少五个角度的照片及该商标的描述。

　　5)如拟注册商标包含非英文的字眼，需提供英文译本。

6)如有指定颜色，需提供10份商标图案(如没有指定颜色，则暗示包含所有颜色);

Documents required for application for Polish trademark registration

 

Note : All documents must be written / displayed in English. If not in English, notarized English versions must be submitted.

 

1. Simple proxy ;

2. Name, nationality, occupation and address of the applicant ; if the applicant is a partnership, it is necessary to provide all partners ' personal data. Where the applicant is a body corporate, a registration certificate or business registration certificate or business license is required. If the applicant has a business address in Poland, a detailed business address is required.

3. Commodities or services to be used and related categories ( by international classification ).

4. Information on proposed registered trademarks :

1 ) If the word to be registered is not English, specify the language and English translation of the word ;

2 ) If the registered trademark is a pattern, it is necessary to simply explain the pattern and 10 pattern samples ;

3 ) In the case of three-dimensional pattern, it is necessary to provide multiple samples of pattern made in two-dimensional and multi-angle ;

4 ) If the proposed registered trademark is full of sample goods or packaging styles, photographs of the trademark at least five angles and descriptions of the trademark are required.

5 ) An English translation is required if the proposed registered trademark contains non-English words.

6 ) If there is a specified color, it is necessary to provide 10 trademark designs ( if there is no specified color, it implies that all colors are included ) ;
